Admission Requirements
- High-school diploma, GED, or foreign equivalent (evaluation for international transcripts)
- Valid government-issued photo ID and SSN / ITIN
- Current immunization record, TB clearance, and physical exam
- Clear Live Scan background check (required by externship sites)
- 18 years of age or older
- CPR/BLS card (can be added during the program)
Admission Procedure
- 1.Complete the CPT-I application and pay registration fee.
- 2.Submit diploma/GED and government ID for verification.
- 3.Attend a brief program orientation and uniform fitting.
- 4.Provide physical exam, immunization record, and TB test results.
- 5.Complete Live Scan and receive externship assignment.

Tuition & Materials
$1,985.00
Included: Registration fee, Phlebotomy Essentials textbook & workbook, Live Scan processing, liability insurance, one set of scrubs & ID badge, externship placement, and the state certification application fee (first attempt).
Grading & Completion
75% minimum on all quizzes and final written exam. Successful completion of 50 unaided venipunctures & 10 skin punctures during externship. Completion documents submitted to CDPH; graduates then apply for CPT-I license.
